What you need to upload.
-
✺
CV
An up-to-date CV which clearly outlines any gaps in your employment history. We’ll need to be able to see clear evidence of relevant work in your field, so please ensure it’s appropriate to your application with us.
-
✺
DBS Check
If you’re already working for the NHS, it’s likely you already have a DBS check. If you don’t, you can get one from the Disclosure & Barring service here. Please upload your DBS check to your Lantum account, and keep it up-to-date.
-
✺
Proof of Address
We need proof of address, dated within the last three months. Valid forms of proof of address include bank or credit card statements, utility bills, mortgage statements or council tax letters.
-
✺
Photo ID
We need two forms of Photo ID. Valid forms of Photo ID include your Driving License or Passport. If you don’t have one of these forms of ID, we can accept a Birth Certificate. However, we must have at least one of these and your Birth Certificate as a minimum.
-
✺
Medical Questionnaire
To make sure you’re working safely and in appropriate environments, we need you to complete our Medical Questionnaire. Complete this here and upload the completed form to your Lantum account under the Occupational Health section.
-
✺
Right to Work
If you require a visa to work in the UK, or need to supply other proof of your right to work status, you’ll need to include this as part of your application. Please note we are unable to take sponsorship applications via this platform.
-
✺
References
We need two valid employer references for you to work with us. Please share their details via Lantum. To find out more about the Reference process, click here.
-
✺
Annual Registration Form
We need you to confirm that you understand the importance of confidentiality when working with us, and that you both are currently up-to-date with your training requirements, and will continue to stay up-to-date with them as you continue to work with us. We also need information on any criminal records you may have. Please complete this form, and sign and upload the version emailed back to you under the Extra Documents section of your Lantum account.
-
✺
Smartcard
You’ll need to enter your Smartcard number to access our systems quickly and easily when you’re working with us. Please upload this as part of your application.
-
✺
Bank Details
While not entering your Bank Details won’t stop you from getting work with us, we’d recommend doing this before your first shift to make the payment process seamless. Everyone who works with us is paid on a self-employed basis, and Lantum allows you to take advantage of RocketPay’s speedy payments.

Not as such, though we do offer flexible work on a locum basis - and we can also help you find permanent salaried roles, if that’s what you’re looking for.

We’re part of our local GP federation, so we deliver a range of NHS-funded programmes throughout the year, such as Flexible Pools - and rather than working on a profit-led basis, we channel our revenues back into General Practice, reinvesting any profits back into our PCNs.
-
Every April-May, we ask every member of staff who works with us to confirm their data is up to date on Lantum as part of our Annual Compliance Check. This helps us to ensure all our staff are working safely, and helps us to stay on top of our CQC compliance. If you’ve been asked to take part, please log into Lantum and confirm your documents are up-to-date to regain access to your account.

We can only offer sponsorship in certain instances, and only at set times of the year when we have capacity to do so. Currently, our books are closed to new applicants, but please check back again at a later date when we may reopen for applications.
